Schedule: Monday - Friday 8 am - 5 pm

What You’ll Do

• Perform spiritual bereavement assessments of patients and families/caregivers in a timely manner and ensure identified needs are incorporated into the patient’s care plan
• Provide direct spiritual care and bereavement counseling to patients and families/caregivers
• Demonstrate exceptional service to patients, families, referral sources, co-workers, and other stakeholders. Assist team members as needed to ensure consistent high-quality service
• Accurately document patient services to reflect the plan of care
• Participate in group meetings to coordinate care and discuss interdisciplinary involvement
• Provide consultation, education, and support on spiritual care and bereavement counseling to the interdisciplinary team
• Participate in continuous care and on-call duties as necessary
• Assist in supervising spiritual care volunteers when assigned to patient families/caregivers
• Lead the development and implementation of the bereavement care plan, including the provision of services up to one year after the patient's death

Qualifications

• Graduate of an accredited seminary or theology school, with certification or ordination from a religious faith group; Clinical Pastoral Education (CPE) preferred
• Previous hospice experience with death and dying strongly preferred
• Comfortable in an interfaith setting and accepting of diverse lifestyles, cultures, beliefs, and values
• Valid driver’s license and proof of insurance required
