Job description

Hospice Certified Nursing Assistants (Healthcare)

Traditions Home Health Services is currently seeking compassionate, skilled, reliable, well-polished, and experienced Hospice-trained Certified Nursing Assistants to assist our client on Hospice residing in the Milton area.

Shifts available:
Days: Monday through Sunday, 8am-8pm (12-hour shifts)
Overnight: Monday through Sunday, 8pm-8am (12-hour shifts)

CNA Job Responsibilities:

1. Assist with daily living activities, including toileting, incontinence care, bathing, dressing, grooming, nail care, oral hygiene, and skin integrity maintenance. Document care and findings.
2. Prepare healthy, fresh meals according to the client's care plan, if required, and ensure adequate fluid intake, documenting intake throughout the shift.
3. Adhere to infection control protocols.
4. Monitor client's skin integrity, perform daily skin checks, and document findings.
5. Administer medication reminders and assist with the application of eye drops, ointments, creams, and hospice medications.
6. Continuously monitor and assist with all transfers and ambulation, as the client is a fall risk.
7. Assist with transfers and operate mechanical lifts if required.
8. Provide proficient dementia and hospice care.
9. Document shift activities digitally and hourly using a smartphone.
10. Report any concerns to the Traditions Home Health Services Office Support Team promptly.
11. Collaborate respectfully with the hospice agency and staff as colleagues.
