Job summary

Aveanna Healthcare is seeking a Hospice Certified Nursing Assistant (CNA) to join our home health care team in Tennessee. This full-time role provides essential personal care and basic medical support to patients in their homes.

You will follow a schedule of Monday–Friday 8AM–5PM across multiple counties, and you must maintain accurate clinical records, communicate with the care team, and ensure patient safety with compassionate, professional care.

Qualifications

• Must be 18+ with HS diploma or GED.
• Valid Certified Nursing Assistant accreditation.
• Completion of competency testing for HHA.
• Valid driver's license and acceptable MVR.
• Valid CPR certification.

Responsibilities

• Perform personal care tasks including hygiene, grooming, feeding, ambulation and health monitoring under supervision.
• Assist with home management such as light housekeeping, laundry, meal prep, and care-related errands.
• Maintain clinical records according to Aveanna policy and regulatory requirements.
• Report changes in client condition to the interdisciplinary team.
• Communicate professionally with clients, families, and staff.

Aveanna Healthcare has grown tremendously during the past year through a series of exciting acquisitions and we are currently looking for a Hospice Certified Nursing Assistant (CNA) Full Time to join our dynamic home health care team. In this position, you make it possible for patients to recuperate from illness or injury in the comfort of their own home.

Schedule

Monday through Friday 8AM-5PM

Territory

Hickman, Williamson, Maury, Davidson and Dickson Counties

Essential Job Functions

• Perform personal care tasks including assistance with basic personal hygiene and grooming, feeding, ambulation, medical monitoring, and health care related tasks under the supervision of the Clinical Coordinator.
• Perform home management functions such as light housekeeping, laundry, bed making, meal prep, service and cleaning
• Maintain clinical record documentation according to Aveanna policy and procedures as well as state and federal regulations
• Report and Document services provided and changes in client's condition or family situation to the appropriate interdisciplinary team member
• Communicate verbally with clients, client family members, and Aveanna staff in a professional manner
• Ensure client safety while maintaining a clean and safe environment

Requirements
