Job summary

Interim HealthCare is hiring a Hospice CNA in Rochester, MN to provide one-on-one home care to patients. This role emphasizes compassionate, patient-centered care with a strong emphasis on independence and meaningful relationships.

The position offers daytime hours Monday through Friday, with mileage reimbursement for travel between visits and tuition discounts through Rasmussen University.

Qualifications

• High school diploma (or equivalent) and active Nursing Assistant Certification in Minnesota.
• CPR certification.
• Valid Minnesota driver’s license, auto insurance and transportation.

Responsibilities

• Care for seniors and those with six months or less to live.
• Assist with daily activities: bathing, toileting, dressing, grooming, eating and nutrition.
• Assist with ambulation, transfers and ROM exercises.
• Check vitals, monitor intake/output and document changes.
• Collaborate with hospice nurses and aides to follow care plans.
• Observe patient and notify supervisor of changes in care or equipment.

Job Responsibilities

• Care for seniors and those with an illness, injury or chronic disease that have six months or less to live
• Help with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating and nutritional intake
• Assist with ambulation, transfers and range of motion exercises
• Check vitals, measure intake/output, monitor blood glucose and document any changes
• Work with a team of Hospice nurses and aides to ensure a patient’s care plan is followed
• Observe patient and notify supervisor of issues requiring a change in care, medication or medical equipment

Requirements

• High school diploma (or equivalent) and active Nursing Assistant Certification in Minnesota
• CPR certification
• Valid Minnesota driver’s license, auto insurance and transportation
• Compassionate nature, good communicator and ability to lift 50 lbs.
