Job summary

Hospice of the Chesapeake is seeking dedicated Certified Nursing Assistants to join our compassionate team in Waldorf, MD. As a CNA, you will help provide comprehensive patient care to individuals with life-limiting illnesses, supporting patients and families through the end-of-life journey.

Under RN supervision, you will perform essential care activities, monitor changes in patient status, and coordinate with the interdisciplinary team to maintain comfort and dignity.

Qualifications

• Current Maryland CNA certification.
• Minimum of one year of experience in a health care setting.
• Valid driver’s license and reliable transportation.
• Ability to meet the physical demands of the position.

Responsibilities

• Under RN direction, perform care tasks per plan, including turning, transporting, catheter care, bathing, skin care.
• Report status changes to the RN Case Manager or designee to keep team updated.
• Identify and recommend medical equipment to support patient comfort and dignity for families.
• Provide compassionate care throughout the end-of-life transition.
• Follow CNA standards of practice and help meet quality goals.
• Other goals and duties as described in the job description.

Want to join our Team? We are currently looking for dedicated Certified Nursing Assistants to join our team. As a Certified Nursing Assistant, you will assist with providing care for those with life-limiting illnesses, helping patients and their caregivers navigate end-of-life care with compassion.

Main Responsibilities

• Under the direction of an RN, perform all required interventions and/or procedures to provide total patient care in the clinical setting according to the plan of care, including turning, transporting, catheter care, bathing, and skin care.
• Report changes in the patient’s medical, emotional, spiritual, or psychosocial status to the RN Case Manager or designee to ensure that the interdisciplinary care team remains up to date.
• Using your hands-on, in person experience with each patient, identify and recommend medical equipment and supplies that will help our patients maintain comfort and dignity and ensure families and caregivers are also able to provide this care.
• Provide compassionate care throughout the end-of-life transition.
• Follow nursing assistant standards of practice and meet or exceed HOC quality goals to ensure the best possible outcomes for the patient and family.
• Other specific goals and duties as communicated in your job description and performance goals.

Qualifications

• Current Maryland CNA certification.
• Minimum of one year of experience in a health care setting.
• Valid driver's license and acceptable driving record and reliable transportation
• Ability to meet the physical demands of the position with or without reasonable accommodation.
