Job summary

Elysian-Hospice in San Antonio, TX seeks a Certified Nursing Assistant (CNA) to provide personal care and supportive services to hospice patients in their homes or care settings. The CNA works under the supervision of a nurse, assisting with ADLs and reporting changes to the clinical team.

The role requires bilingual English/Spanish, compassion, reliability, and the ability to work as part of the hospice care team.

Qualifications

• Current state CNA certification required.
• Experience in Hospice, home health, long-term care, or healthcare preferred.
• Strong communication and patient care skills.
• Ability to follow care plans from nursing staff.

Responsibilities

• Provide personal care (bathing, grooming, dressing, toileting, mobility).
• Assist with activities of daily living per care plan.
• Document patient care and observations accurately and timely.
• Report changes in patient condition to supervising nurse.
• Maintain a safe, clean patient environment and follow infection control.

Location: San Antonio, TXJob Summary:The Certified Nursing Assistant (CNA) provides personal care and supportive services to hospice patients in their place of residence in accordance with the patient’s plan of care. The CNA works under the supervision of a nurse and supports patients with activities of daily living while promoting patient comfort, dignity, and safety. The CNA plays an important role in supporting hospice patients and families by assisting with daily care needs and reporting changes in patient condition to the clinical team.We are seeking a CNA who is bilingual in English and Spanish.
