Job summary

Touchstone Health Hospice seeks a Certified Nurse Aide to provide compassionate, in-home care for hospice patients in the San Antonio area. You will assist with activities of daily living, monitor vital signs, and provide emotional support to patients and families.

Under the supervision of a licensed nurse, you will document care in the EMR, participate in ongoing training, and help patients maintain dignity and comfort during this meaningful stage of life.

Qualifications

• Current Texas Nurse Aide certification
• Completion of state-approved CNA training program
• High school diploma or equivalent
• 1+ year experience preferred; hospice experience a plus
• Strong communication and interpersonal skills
• Reliable transportation with valid driver’s license and auto insurance

Responsibilities

• Provide personal care services under the supervision of a licensed nurse
• Assist patients with bathing, grooming, dressing, mobility, toileting, and feeding
• Measure and record vital signs (temperature, pulse, respiration, blood pressure)
• Observe and report changes in patient condition to supervising staff
• Assist with ambulation, transfers, and positioning to promote comfort
• Maintain accurate EMR documentation of care provided
• Provide emotional support and companionship to patients and families
• Participate in in-service training and continuing education

About the Role

As a Certified Nurse Assistant with Touchstone Health's Hospice team in San Antonio, you'll provide hands-on, compassionate care to hospice patients in the comfort of their own homes. You'll support activities of daily living, monitor vital signs, and offer the kind of steady, personal presence that helps patients maintain dignity and comfort at one of life's most meaningful stages. This role is a natural fit for someone who wants to build real relationships with the patients and families they care for, not just check tasks off a list.

Key Responsibilities

• Provide personal care services in accordance with each patient's plan of care and under the supervision of a licensed nurse
• Assist patients with bathing, grooming, dressing, mobility, toileting, and feeding

Requirements

What We're Looking For

• Current certification as a Nurse Aide in the state of Texas
