Job summary

Cantex Continuing Care Network is seeking a bilingual CNA to provide personal care and support to hospice patients in their home or facility setting in San Antonio, TX. The CNA works under a nurse supervision to assist with ADLs and ensure comfort, dignity and safety.

This PRN position requires reliable, compassionate caregivers fluent in English and Spanish, able to follow patient plans of care and communicate changes to the clinical team.

Qualifications

• Current state certification as a CNA required
• Hospice/home health/long-term care experience preferred
• Strong communication and patient care skills
• Ability to read, write, and communicate effectively in English

Responsibilities

• Provide personal care including bathing, grooming, dressing, toileting, and mobility assistance
• Assist with activities of daily living per the plan of care
• Document patient care and observations accurately
• Report changes in patient condition to supervising nurse
• Promote patient safety, comfort, dignity, and independence

Job Summary:

The Certified Nursing Assistant (CNA) provides personal care and supportive services to hospice patients in their place of residence in accordance with the patient's plan of care. The CNA works under the supervision of a nurse and supports patients with activities of daily living while promoting patient comfort, dignity, and safety. The CNA plays an important role in supporting hospice patients and families by assisting with daily care needs and reporting changes in patient condition to the clinical team.

We are seeking a CNA who is bilingual in English and Spanish. This role requires strong patient care skills, compassion, reliability, and the ability to work effectively as part of the hospice care team.

Qualifications

• Current state certification as a Certified Nursing Assistant (CNA) required
• Hospice, home health, long-term care, or healthcare experience preferred
• Strong communication and patient care skills
• Ability to follow patient care plans and instructions from nursing staff
• Ability to work independently in a home or facility setting
• Reliable transportation, valid driver's license, and current auto liability insurance
• Ability to read, write, and communicate effectively in English

Essential Functions
