Job summary

Cantex is seeking a compassionate Hospice Registered Nurse (RN) to provide skilled nursing care in patients' homes in San Antonio, TX. You will coordinate care with the interdisciplinary team, physicians, and families, and perform comprehensive assessments to develop personalized plans of care.

Ideal candidates hold an RN license with hospice experience preferred, strong documentation skills, and reliable transportation. English and Spanish bilingual abilities are a significant plus.

Responsibilities

  • Perform initial and ongoing comprehensive nursing assessments for hospice patients.
  • Develop and update individualized hospice plans of care for quality and continuity of care.
  • Coordinate patient care with IDT, physicians, patients and families.
  • Educate patients and families on disease, symptoms, and end-of-life care.
  • Determine scope and frequency of nursing services based on patient needs.
  • Supervise and provide direction to aides and LVN/LPN staff.

Location: San Antonio, TXJob Summary:The Hospice Registered Nurse (RN) is responsible for providing skilled nursing care to hospice patients in their place of residence and coordinating patient care with the Interdisciplinary Team (IDT), physicians, patients, and families. The RN performs comprehensive assessments, develops and manages hospice plans of care, and ensures quality patient care and regulatory compliance throughout the hospice care process.This role requires strong clinical assessment skills, care coordination experience, and the ability to work independently in a hospice field setting while collaborating with the interdisciplinary care team.
