Job summary

Elysian-Hospice is seeking a dedicated Certified Nursing Assistant (CNA) to provide compassionate personal care to hospice patients in their homes or designated facilities in line with each patient’s care plan.

You will work under the supervision of a nurse, assisting with daily living activities and reporting changes in patient condition to the clinical team. Strong empathy, reliability, and teamwork are essential for success.

Qualifications

  • Current state CNA certification required.
  • Hospice, home health, long-term care or healthcare experience preferred.
  • Ability to read, write and communicate effectively in English.

Responsibilities

  • Provide personal care including bathing, grooming, dressing, toileting and mobility assistance.
  • Assist patients with activities of daily living according to the plan of care.
  • Document patient care and observations accurately and timely.
  • Report changes in patient condition to supervising nurse.
  • Maintain a clean and safe patient environment.
  • Follow infection control and safety procedures in all patient care activities.
  • Assist with transfers and mobility as needed.

The CNA works under the supervision of a nurse and supports patients with activities of daily living while promoting patient comfort, dignity, and safety.
