Job summary

Swan Hospice is seeking a Certified Home Health Aide (CHHA) with hospice experience to join our team in New Jersey. The role supports patients in skilled nursing facilities, assisted living facilities, and private homes with compassionate care.

Ideal candidates hold a NJ HHA license, have at least one year of hospice experience, and possess dependable transportation to cover assigned visits across care settings.

Qualifications

• HHA license in NJ is required.
• Minimum of one year hospice experience.
• Ability to understand and embrace hospice philosophy.

Responsibilities

• Assist with prescribed exercises and ROM activities as taught by the Hospice Nurse.
• Provide assistance in toileting, positioning, transfer, turning, and maintaining bed-bound residents/patients.
• Assist with ADL activities, personal hygiene, grooming, mouth, skin care, and nails.
• Assist or remind patient to take medications in accordance with regulations.
• Light housekeeping of the patient’s unit or room, bed linen changes, and patient laundry.
