Job summary

Harbor Hospice is seeking a dedicated RN/LPN Case Manager for full-time weekday coverage with weekend/holiday rotation in Michigan. You will provide skilled nursing care, develop patient care plans, and lead weekly interdisciplinary team meetings to ensure optimal home care outcomes.

Responsibilities include charting, medication management, IV starts, venipuncture, and care coordination with families and clinicians. Valid MI license and CPR are required.

Qualifications

• Licensed RN or LPN with a current Michigan license.
• Current CPR certification required.
• Valid Michigan driver's license and reliable insured vehicle.

Responsibilities

• Provide skilled nursing care per physician orders and nursing policies.
• Develop and overseePlans of Care for Home Care patients.
• Lead weekly interdisciplinary team meetings and coordinate care.
• Perform GIP on/off audits and manage relevant charting.
• Monitor medications, IV starts, venipuncture, and port access as needed.
