Hospice of Muskegon County is seeking an RN or LPN Case Manager for Home Care in Muskegon, MI. This full-time position includes day shift, with weekend and holiday rotation, and requires active Michigan license.

The role involves creating and overseeing plans of care, leading interdisciplinary team meetings, coordinating admissions, and ensuring compliance with nursing regulations and accreditation standards.

Qualifications

• RN or LPN with degree from an accredited nursing school and current license in Michigan.
• CPR certification required.

Responsabilités

• Lead weekly interdisciplinary team meetings and participate in care decisions.
• Assess patient appropriateness for hospice services and develop plans of care.
• Coordinate admissions and assist with discharge planning for Home Care patients.
• Perform venipuncture, IV starts, CADD pumps as needed, and related nursing duties.
• Complete charting, including daily GIP charting and regulatory compliance.
• Manage medications, e-box prescriptions, and ensure proper orders in Home Works.
• Educate patient/family on DME usage and care procedures.
• Orient staff and volunteers and provide in-service training as needed.
