Job summary

Harbor Hospice is seeking an RN Case Manager to join our Home Care team in Michigan. The role is full-time with day shifts and rotating weekends/holidays.

You will provide skilled nursing care per physician orders and our regulatory standards, and you will develop and oversee Plans of Care for each patient under our program. Qualifications include an active Michigan RN license, CPR certification, and a valid Michigan driver’s license with access to a reliable vehicle.

Qualifications

• Must be a Registered Nurse with a degree from an accredited nursing school.
• Current license to practice as a Registered Nurse in Michigan.
• CPR certification must be current.

Responsibilities

• Provide skilled professional nursing care as prescribed by the physician and per Harbor Hospice policies.
• Create and oversee Plans of Care for each patient admitted to Home Care.
