Residential Home Health and Residential Hospice in Troy, MI is seeking a Hospice Transitional Nurse Liaison to collaborate with field teams and leaders in coordinating patient-centered hospice care. The position requires an Associate degree in Nursing and at least two years of RN experience, with preference for acute care or oncology background.

Qualifications

• Must hold an active RN license in Michigan.
• Two years RN experience, acute care or oncology preferred.
• Familiarity with hospice care processes recommended.

Responsibilities

• Perform initial, comprehensive patient and family hospice assessments.
• Gather demographic and medical history details.
• Orient patients to services and coordinate start-of-care.
• Schedule DME, supplies, and medications.
• Deliver community education to facilities and hospitals.
• Educate patients on hospice care and end-of-life options.
• Support discharge planning and coordinate safe home transitions.
• Determine hospice eligibility and level of care.
• Communicate with clinical team to prevent rehospitalizations.
