Job summary

DayToDay Health seeks a Registered Nurse (RN) specializing in Hospice care to support patients and families in a home or skilled nursing facility setting. You will assess needs and implement care plans while collaborating with families and the healthcare team.

The role requires an active RN license, a nursing degree, and a valid driver’s license, with a preferance for a bachelor’s degree and 2–5 years of hospice experience. Weekend work and travel within the Greater St. Louis area may occur.

Qualifications

• Nursing diploma or associate's degree with <2 years of experience.
• RN license and valid driver’s license required.
• Bachelor's degree and 2-5 years of relevant experience preferred.

Responsibilities

• Assess patient needs, develop and implement individualized care plans, promote cultural sensitivity, evaluate patient condition changes, and communicate effectively with patients, families, and the healthcare team.
