Job summary

Metropolitan Jewish Health System, Inc. is seeking a Hospice Care Planner to evaluate and establish care plans for hospice referrals and to facilitate admissions from the hospital to appropriate hospice programs.

You will work as a key member of the business development and marketing team, building and maintaining relationships with physicians, social workers, case managers and discharge planners within the medical center.

Qualifications

• Licensed with current Registration to practice as Registered Professional Nurse in NYS is required.
• Bachelor's Degree preferred.
• Must have a minimum of two years experience in Hospice nursing with proficiency in physical assessment and interviewing skills.
• Must be client-service focused with excellent written and verbal communication skills.
• Advanced critical thinking and problem solving skills, plus the ability to work independently is essential.
• Bilingual Spanish/English is required in some locations.

Our groundbreaking Hospice and Palliative Care programs offer a broad range of services in both in-patient facilities and the community to help patients and their families ease the burden of managing a life-limiting illness or end-of-life care.

Working on-site at a major medical center, the Hospice Care Planner is responsible for evaluating and establishing a plan of care for patients referred for Hospice Care services.
