Job summary

Lehigh Valley Health Network (LVHN) is seeking a Home Care and Hospice Patient Care Manager to lead a dedicated hospice care team. The role ensures and/or provides patient care per the RN scope, with 24-hour accountability for directing and managing clinical operations within the assigned unit in Allentown, PA.

The successful candidate will supervise staffing, coordinate resources, and promote patient safety by following policies, mentoring staff, and supporting the Professional Practice Model.

Qualifications

• Bachelor’s Degree in Nursing and active RN license.
• 3 years of RN experience in an acute care hospital setting.
• BLS certification required; ability to multitask and prioritize work.

Responsibilities

• Oversee patient care and 24-hour accountability in the designated care area.
• Lead care delivery team; support unit and hospital goals; manage staffing and resources.
• Foster a culture of safety by following policies and reporting events; mentor staff and participate in professional practice.
