Job summary

AMOREM is seeking a Clinical Liaison who combines strong nursing skills with the ability to build trusted relationships, educate others, and coordinate care during some of life’s most important moments. The role connects hospital providers, patients, and families with AMOREM’s hospice services.

The ideal candidate is a confident clinician, a clear communicator, and a proactive relationship builder who supports informed decisions and timely transitions to hospice care within hospital settings.

Qualifications

• Bachelor’s degree in nursing is required.
• Minimum of two years of nursing experience; hospice experience preferred.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently and coordinate effectively across teams.
• Commitment to compassionate care and expanding access to hospice services.

Responsibilities

• Serve as AMOREM’s primary liaison and organizational ambassador within assigned hospital.
• Develop and maintain relationships with physicians, case managers, social workers, discharge planners, nurses, and other healthcare professionals.
• Provide education about hospice services and timely referrals for eligible patients.
• Coordinate hospital referrals and transfers for routine hospice care and general inpatient hospice care.
• Visit hospitalized hospice patients to support continuity of care and communication among patients, families, hospital staff, and the hospice team.
• Respond promptly to referrals, questions, and requests, and follow through until needs are addressed.
• Demonstrate sound clinical judgment and ability to manage competing priorities.
• Participate in Quality Assurance and Performance Improvement activities.
