AMOREM is seeking a Clinical Liaison who blends nursing expertise with strong relationship-building to connect hospital providers, patients, and families with hospice services.

In this role you will educate clinicians on hospice options, coordinate referrals and transitions to care, and serve as a trusted contact to support timely, patient-centered decisions. You will work with an interdisciplinary team to ensure compassionate, high-quality end-of-life care.

Qualifications

• Bachelor’s degree in nursing (BSN).
• Minimum of two years’ nursing experience; hospice experience preferred.
• Strong clinical assessment and communication skills.
• Ability to build collaborative relationships with healthcare professionals.
• Ability to work independently and coordinate across teams.

Responsabilités

• Serve as primary liaison within assigned hospital.
• Educate staff and explain hospice services and referrals.
• Coordinate referrals and transitions to care.
• Document findings accurately in EMR.
• Collaborate with interdisciplinary team on care plans.
• Respond promptly to referrals and inquiries.
• Support continuity of care and communication among patients, families, and hospital staff.
• Uphold AMOREM’s mission and participate in quality improvement.
