Responsibilities

• Coordinate with physicians, staff, and non-Kaiser providers regarding patient care.
• Develop plans of care and discharge plans in conjunction with physicians.
• Perform daily pre-admission, admission, and concurrent utilization reviews.
• Monitor the progression of the plan of care and facilitate discussions with multidisciplinary teams.
• Educate healthcare team members on utilization and cost containment initiatives.
• Collaborate with patients, families, and staff regarding care provisions.
• Develop, evaluate, and coordinate comprehensive discharge plans.
• Monitor care processes and assist in developing guidelines and care paths.
• Identify opportunities for cost-savings and improvements in quality of care.
• Provide direct patient care as needed to maintain clinical expertise.
