Job summary

North Country Healthcare is seeking a Bereavement Coordinator & Volunteer Liaison to develop, coordinate, implement, and evaluate hospice bereavement and volunteer programs. This role ensures Medicare CoP compliance and supports patients, families, and communities throughout the hospice journey.

The position collaborates with the Social Worker, IDT, and agency leadership, providing emotional support, grief resources, and community education.

Qualifications

  • Bachelor's degree in a related field required or 8 years of experience in the field in lieu of formal education.
  • Certified in Thanatology (CT) via ADEC preferred.
  • Certified Grief Support Practitioner (GSP-C) preferred.

Responsibilities

  • Coordinate and oversee the hospice bereavement program in accordance with Medicare Conditions of Participation.
  • Provide pre-bereavement support and bereavement assessment during active hospice episode with IDT coordination.
  • Maintain bereavement contacts and risk assessments for up to 13 months after death.
  • Coordinate and supervise hospice volunteers and match them to patient needs.
  • Participate in IDT meetings and quality improvement activities.
  • Ensure HIPAA compliance and proper documentation in the EMR.
