Chapters Health System is seeking a Bereavement Counselor to provide individual and group counseling to hospice patients, families, and community members experiencing loss. Qualifications

• Master’s degree required if hired after 5/26/2024.
• Bachelor’s degree preferred if hired prior to 5/26/2024.
• Clear Level 2 background check is required.

Responsibilities

• Provide individual and group counseling and therapy for Hospice patients, family members and community individuals who have experienced the loss of a significant person through grief.
• Document at least two additional attempts to reach an assigned client before closing the case.
• Manage tasks and caseloads assigned, from any departmental program, productively and timely.
• Provide counseling services such as on-call crisis intervention, individual and group, on a flexible schedule including some weeknights and weekends.
• Document all client contacts and care plans accurately according to policy and maintain all required files.
