Affinity Health Management is seeking a Bereavement Coordinator to plan, implement, and maintain a bereavement program for families/caregivers up to one year after a hospice patient’s death. The role includes coordinating grief counseling, developing program services, supporting hospice staff, and maintaining community resources.

The position requires a degree (Bachelor’s or Master’s in Social Work) with experience in grief counseling and hospice philosophy.

Competenze

• Bachelor’s degree or equivalent theological degree; experience with families/caregivers and patients through death/dying issues or Master’s in Social Work with grief counseling qualifications.
• Knowledge of hospice philosophy and good verbal/written communication.
• Licensed driver with insured automobile; CPR certification.
• Background checks required.

Mansioni

• Coordinate grief counseling assessment and delivery (one-to-one, groups, follow-up).
• Plan bereavement program development, expansion, and annual evaluation.
• Provide bereavement support to hospice staff and supervise interns.
• Provide community referrals and maintain resource lists.
• Assist in evaluating services to meet staff support needs.
• Participate in education, meetings, and committees; advance professional knowledge.
• Report mandatory health dept. concerns within required timelines.
• Actively participate in quality improvement teams.
• Perform other related duties as required.
