Qualifications

• Proficient in communicating in English (reading, writing, speaking).
• Knowledge of electronic medical records (EMR).
• Nursing skills per licensed provider and ethical standards.
• Proficient with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
• Ability to read and interpret orders and care plans.
• Adheres to infection control precautions in patient care.
• Completes AdventHealth Hospice Care Orientation and Training as required.

Responsibilities

• Schedule: PRN shifts, objective orientation with Monday–Friday 8am–5pm for first 2 weeks.
• Provide hospice medical care, assessments and education to patients and families.
• Collaborate with licensed physician to order treatment plans.
• Document daily visits accurately in patient records.
• Participate in interdisciplinary team conferences and education sessions.
• Support billing documentation and reporting to Hospice Finance.
