Job summary

AccentCare Inc. is seeking a Hospice Aide Coordinator in Miami to lead schedules and visits for Hospice Aides while collaborating with nursing leadership to ensure alignment with patient care plans.

The role supports field training, competency reviews, and interdepartmental communication. The position requires at least two years in hospice care, relevant certifications, and proficiency with Microsoft Office and EMR systems.

Qualifications

• High school diploma required; Nurse’s Aide certification or CHHA/CGNA preferred; CPR certification may be required in Georgia.
• At least 2 years of hospice care experience; CHAPNA certification preferred.
• Proficiency in Microsoft Office and EMR systems; strong written and verbal communication; ability to follow directions.
• Reliable transportation, valid driver’s license; able to meet physical demands (standing, walking, lifting up to 200 lbs with assistance)
• Ability to work in varied conditions including heights, dampness, chemicals, dust, radiation, and noise,
• Demonstrates maturity, compassion and ability to handle emotional and physical demands; aligns with TRUE HOPE values.

Responsibilities

• Coordinate and assign Hospice Aide schedules and visits with nursing leadership to align with care plans and staffing needs.
• Mentor and liaison for HAs; support field training, orientation, competency reviews; participate in committees such as QAPI.
• Oversee medical supply management including ordering, distribution, cost control and storage.
• Provide direct patient care as needed (hygiene, mobility, elimination, simple procedures) and document per care plan.
• Collaborate with interdisciplinary team, clinical staff, facilities and families to ensure coordinated care.
• Take on additional responsibilities such as carrying a patient caseload or supporting other licensure scope as assigned.
