Qualifications

• Strong written and verbal communication and ability to follow directions.

Responsibilities

• Coordinate and assign Hospice Aide schedules and visits with nursing leadership.
• Mentor and liaison for Hospice Aides; support field training and orientation.
• Oversee medical supply management and documentation per care plans.
• Provide direct patient care as needed under the interdisciplinary plan of care.
• Collaborate with interdisciplinary team to ensure coordinated compassionate care.
