AccentCare, Inc. is seeking a Hospice Aide Coordinator in Miami to join a purpose-driven care team.

This role coordinates Hospice Aide schedules, supports field training, and ensures alignment with patient care plans under a caring, in-office environment. Ideal candidates have a nursing assistant certification and hospice experience, with strong communication skills and proficiency in Microsoft Office and EMR systems.

Qualifications

• High school diploma required with current certification as a Nurse’s Aide, CHHA, or CGNA; CPR certification preferred.
• At least 2 years of hospice care experience; CHAPNA preferred.
• Proficiency in Microsoft Office and EMR systems; strong written and verbal communication; follow directions.
• Must have reliable transportation, valid driver's license, and be able to handle physical demands (lift up to 200 lbs with assistance).
• Able to work in varied conditions including heights, dampness, chemicals, dust, radiation, and noise.

Responsibilities

• Coordinate and assign Hospice Aide schedules and visits with nursing leadership.
• Mentor and liaison for HAs; support field training and competency reviews.
• Oversee medical supply management including ordering and cost control.
• Provide direct patient care as needed per interdisciplinary plan of care.
• Collaborate with interdisciplinary team, facilities, and families to ensure coordinated care.
• Take on additional responsibilities as assigned.
