Optimal Care seeks a compassionate Hospice Aide/Homemaker to support patients at home, helping with daily living tasks and comfort during life's meaningful moments.

You will work closely with nurses and the interdisciplinary team, building trusting relationships with patients and families while maintaining dignity and empathy. This role involves travel within the service area across Highland, Fenton, Clarkston, Lake Orion, Troy, Rochester Hills, and surrounding communities.

Qualifications

• Must hold current Nurse Aide certification and be in good standing.
• 75 hours classroom and 16 hours practical training completed.
• Reliable transportation, valid driver's license, and auto insurance.

Responsibilities

• Provide personal care including bathing, grooming, toileting, and mobility.
• Assist with meals, oral hygiene, and daily activities.
• Support patients and families while collaborating with nurses.
• Document care and report changes to the care team.
