Job summary

Community Hospice & Palliative Care in Jacksonville, FL is seeking a Hospice Aide to provide compassionate, hands-on care to terminally ill patients across care sites, including weekends and holidays. The aide supports comfort, dignity, and safety under the established plan of care.

This full-time shift is 7 p.m. – 7 a.m., 36 hours per week, with a $1000 sign-on bonus. Requires Florida CNA license, CPR/BLS, reliable transportation, and ability to work as part of an interdisciplinary team.

Qualifications

• Understanding of personal care principles and hospice care standards.
• Ability to communicate effectively with patients, families, and team members.
• Demonstrated ability to work as part of an interdisciplinary team.
• Strong organizational and documentation skills.

Responsibilities

• Provide personal care services in accordance with the plan of care and hospice regulations.
• Assist with patient ambulation and exercise routines.
• Perform basic procedures as directed by licensed clinicians.
• Observe and report changes in the patient’s condition to a licensed nurse.
• Complete timely and accurate documentation per policy.
• Educate family or caregiver on proper patient care techniques.
• Maintain confidentiality of all patient information.
• Participate in interdisciplinary team meetings and other required sessions.
• Complete a minimum of 12 hours of job-specific training annually.
