Job summary

Affinity Hospice Management LLC is seeking a dedicated Hospice Aide to support patients under RN supervision. You will perform personal care, assist with activities of daily living, and promote comfort in line with each patient’s plan of care.

The role requires a state-approved CNA course, NAC and CPR certifications, a Food Handlers Permit, and a high school diploma or GED. Prior hospice/home health experience is preferred; strong communication and confidentiality are essential.

Qualifications

• Completion of a state approved certified nursing assistant course.
• NAC certification is required.
• Current Healthcare Provider CPR certification is required.
• Current Food Handlers Permit is required.
• High school diploma or GED is required.
• Ability to read and follow written instructions and document care given.
• Prior hospice/home health experience preferred (1 year).
• Basic computer skills to document in EMR.

Responsibilities

• Perform personal care tasks as assigned and respond to patient requests promptly.
• Maintain positive communication with patients, families and team members.
• Ensure safety and proper use of equipment; assist with transfers and mobility.
• Provide bathing, oral care, dressing and assistance with feeding as assigned.
• Support bed care, linen changes and light housekeeping tasks.
• Report changes in patient condition to RN Case Manager promptly.
• Observe and document patient condition and care provided in EMR.
• Assist with activities of daily living and respite for family when on-site.
