Job summary

Heart’n Soul Hospice in Seattle, WA is seeking a dedicated Hospice Aide (CNA) to join our compassionate team. You will provide direct patient care under RN supervision and carry out the Plan of Care with empathy and integrity.

Responsibilities include safe transfers, documenting changes, infection control, and supporting patients and families with respectful communication. A CNA license, high school diploma or GED, and at least one year of relevant experience are required.

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA certification.
• Current CNA license is required.
• At least one year experience as a Hospice Aide or Nursing Assistant in a relevant setting.
• Demonstrates compassion and interest in welfare of ill and elderly.

Responsibilities

• Provide direct patient care under RN direction and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report and document changes and care provided to ensure continuity of care.
• Follow infection control principles and maintain hygiene standards.
• Communicate respectfully with patients, families and staff.
• Submit all documentation timely and accurately.
• Perform other duties as required and ensure compliance with laws and policies.

Hospice Aide - CNA

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
