Responsibilities

The Hospice Aide provides personal care and related services in the home for the sick or disabled, including assisting the patient with at least 2 activities of daily living (ADLs). As a Hospice Aide, you will:

• Assist with personal hygiene and at least one other ADL
• Assist patients with medication intake and prescribed exercises
• Perform incidental household services essential to patient health care
• Follow the Plan of Care and update it during visits
• Ensure the use of the 4Ms (What Matters, Medications, Mentation, Mobility) and provide Age-Friendly Care

Qualifications

• Must be a certified nurse’s aide (CNA)
• Valid driver’s license, good driving record, and reliable vehicle
• High school diploma or GED preferred
• Ability to work within a multidisciplinary team and in field situations
