Job Description
As the Hospice Aide, you are a paraprofessional and valued member of the interdisciplinary care team. You will work under RN supervision, focusing on quality, continuity, and safe patient care. Your responsibilities include timely and accurate documentation of care.
Job Responsibilities
- Provide patient care including bathing, nail/skin care, dressing/undressing, dental and other personal hygiene while maintaining a sanitary environment
- Communicate positively, respectfully, and confidentially with patients and families
- Take and record vital signs, assist with self-administered medications
- Plan and prepare nutritious meals, including shopping and feeding as needed
- Assess and report patient’s condition and changes to the RN Case Manager
- Assist in ambulation, exercise, transfers, positioning, and support therapy activities

Minimum Qualifications
- At least 18 years of age
- Active Certified Nursing Assistant (CNA) certification as per state requirements
- Valid driver’s license and insured vehicle in good condition
