Pay Range: $17.00 - $22.00/hr. + $5K Bonus

Responsibilities:

1. Follow the interdisciplinary plan of care, be alert to the patient's needs and concerns.
2. Provide personal care, including bathing, dressing, grooming, hair, nail, skin, foot care, and oral hygiene.
3. Assist patients according to the plan of care:
1. Turning
2. Sitting
3. Standing
4. Ambulation
4. Assist with elimination, including:
1. Facilitation of bedpans and urinals
2. Assisting patients to the bathroom
3. Routine Foley care
4. Enemas
5. Meals when necessary
5. Perform simple procedures as an extension of therapeutic services, in accordance with the interdisciplinary plan of care.
6. Observe and report changes in the patient's condition and needs to the Team Director (TD) or Registered Nurse (RN).
7. Perform simple homemaking chores, meal preparation, and/or light housekeeping in the patient's immediate area.
8. Complete required documentation per policy.
9. Participate in continuing education programs.
10. Follow confidentiality policies regarding patients and families.
11. Demonstrate a compassionate attitude toward patients and families, aligned with the organization's philosophy and mission.
12. Attend patient care conferences as requested by the TD.
13. Perform other related duties and special projects as assigned.
14. Perform duties within the scope of current, valid professional licensure/credentials (e.g., RN, SW, Chaplain, MT, HA, etc.). Refer to the specific job description for additional details on essential functions.
