Job summary

Elara Caring is seeking a Certified Nurse Aide to provide hands-on personal care under an RN’s supervision in patients’ homes. You will assist with bathing, grooming, mobility, and daily activities, while following individualized care plans and documenting care provided.

Qualifications

• Current State certificate as a Home Health Aide or Certified Nursing Aide
• Able to frequently lift, push, pull, and support up to 50 pounds
• Must possess a valid driver’s license or have reliable transportation to travel between patient homes

Responsibilities

• Provide hands-on personal care under the direction of a Registered Nurse
• Assist patients with bathing, dressing, grooming, toileting and other daily activities
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure
• Help patients safely walk, transfer, and use mobility equipment
• Follow each patient’s individualized care plan
• Monitor and report changes in a patient’s condition to the care team
• Maintain a clean, safe, and comfortable home environment
• Remind patients to take medications as directed
• Respond to emergency situations and follow safety and infection control guidelines
• Maintain accurate documentation and protect patient privacy

What is Required?

• Current State certificate as a Home Health Aide certification or Certified Nursing Aide certification
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
