Job summary

Elara Caring is seeking a Hospice Certified Nurse Assistant/Hospice Aide to join our team and deliver compassionate home-based care. You will work under the RN to implement the Plan of Care and support patients and families.

Applicants should have 1 year of hospice or nursing assistant experience, a valid driver’s license, and the ability to lift up to 50 pounds. We offer benefits, retirement plans, and opportunities for growth.

  • State hospice aide training program meeting 42 CFR §484.36 and competency evaluation.
  • Ability to frequently lift/push/pull up to 50 pounds for transfers.
  • Valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

  • Provide skilled nursing care per Plan of Care and RN directions.
  • Monitor patient condition and report changes to RN Case Manager and care team.
  • Collaborate with RN Case Manager to update the Plan of Care as needed.
  • Educate patients and caregivers on self-care to support safety and independence.
  • Document medication regimens and maintain accurate profiles.
  • Help prevent avoidable hospitalizations through coordinated patient care.
  • Maintain compassionate, high-quality care with positive communication to patients and families.

Job Description

To continue to be an industry pioneer in delivering unparalleled care, we need a Hospice Certified Nurse Assistant/ Hospice Aide with commitment and compassion.

  • Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.
  • Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.
  • Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.
  • Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.
  • Document medication regimens and maintain accurate, up-to-date medication profiles.
  • Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.
  • Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.
What is Required
  • High School Diploma or GED equivalent required
  • 1 year of experience as a Hospice Aide or Nursing Assistant is required
  • State‑established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation program
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
  • Experience in a home health environment is preferred
