Job summary

St. Croix Hospice is seeking a compassionate Hospice Aide (CNA) to support patients in a home care setting under RN supervision. Responsibilities include personal care, mobility support, meal assistance, and comfort measures to enhance quality of life.

Candidates must hold an active CNA license, be 18+, have a valid driver's license and reliable vehicle, pass background checks and drug screening, and comply with HIPAA.

Qualifications

• Active CNA license in the state of practice is required.
• Must be at least 18 years of age.
• Ability to pass background, drug test, and driving record checks.
• Valid State Driver’s License and access to reliable transportation if needed.
• Physical ability to assist with transfers and lifting up to 50 pounds.

Responsibilities

• Provide personal care including bathing, dressing, oral hygiene, and bed linen changes.
• Assist patients with mobility, transfers, and prescribed exercises.
• Offer meal assistance, feeding, and light household tasks as directed.
• Monitor patient condition and report changes to RN Case Manager.
• Support with medications reminders per state regulations and HIPAA compliance.
• Maintain a clean, safe, and organized patient environment.
• Provide emotional support to patients and respite for family caregivers.

The Hospice Aide provides direct support and personal care for hospice patients, to assist in their day-to-day needs, provide comfort and enhance their quality of life. The Hospice Aide is responsible for following patients’ individual Plan of Care, providing in-person care, maintaining documentation and coordinating with the patient’s RN Case Manager.Essential Functions and SkillsProvide personal care, including bathing, dressing, oral hygiene, and bed linen changes.Assist patients with mobility, transfers, and exercises as instructed by nursing staff.Offer assistance with meals, feeding, and light household tasks as assigned.Monitor and report patient condition changes to the RN Case Manager.Administer routine skin care to prevent breakdown and ensure patient comfort.Support medication reminders per state regulations.Maintain a clean, safe, and organized patient environment.Provide emotional support and respite for family caregivers.Use equipment safely and appropriately for patient care needs.Adhere to all documentation and HIPAA compliance standards.Participate in quality assessment and performance improvement activities.Perform other duties as assigned.Requirements/QualificationsMust have an active Certified Nursing Assistant (CNA) license in the state of practice.Must be at least 18 years of age.Ability to pass background study, consumer report and drug test.Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.Physical RequirementsThe physical requirements described are representative of those that must be met to successfully perform the essential responsibilities of this position.
