Job summary

St. Croix Hospice is seeking a Hospice Aide (CNA) to join our interdisciplinary care team.

You will provide compassionate, hands-on care to patients in their homes, under the direction of a Registered Nurse, supporting personal care and daily living activities to preserve comfort and dignity at end of life. The role requires an active CNA certification in the state, a valid driver’s license, and the ability to travel between patient homes.

Qualifications

• Current CNA certification in the state of practice.
• Ability to provide patient care including personal care and daily living activities.
• Willingness to travel to patient homes throughout the day.

Job Details: Job Location: Eagan, MN 55120, Position Type: Full Time, Travel Percentage: 100%, At St. Croix Hospice, we serve patients and families during one of life’s most important journeys—bringing expert care, dignity, and compassion to every home we visit.

As a Hospice Aide (CNA), you’ll be an essential part of our interdisciplinary care team, delivering compassionate, hands-on care to patients in their homes. Working under the direction of a Registered Nurse, you’ll support patients with personal care and daily living activities, helping ensure comfort, dignity, and the highest possible quality of life at the end of life.

Qualifications: Hospice Aide (CNA) Requirements & Responsibilities:

• Current CNA certification in the state of practice
• Valid driver’s license, clean driving record, and auto insurance
• Provide personal care including bathing, skin/nail care, oral hygiene, dressing/undressing
• Record vital signs and report observations to the care team
• Assist with meal planning, preparation, and feeding as needed
• Promote patient comfort, dignity, and safety
• Travel to patient homes throughout the day and provide care as scheduled
• Maintain accurate documentation of services provided

The hourly base range for this role is $21-25 annual basesalary.An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate inemployee benefits including ourmedical/dental/vision/petinsurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/oreligibility forother compensation.
