Job summary

St. Croix Hospice in Wisconsin is seeking a dedicated Hospice Aide (CNA) to join our interdisciplinary team.

You will provide compassionate, hands-on care to patients in their homes under the direction of a Registered Nurse, supporting comfort, dignity, and quality of life at the end of life. This full-time role requires current Wisconsin CNA certification, a valid driver’s license, reliable transportation, and the ability to travel between patient homes.

Qualifications

• Active CNA certification in Wisconsin.
• Valid driver's license and clean driving record.
• Provide personal care and daily living support.
• Record vital signs and communicate with care team.

Responsibilities

• Provide personal care including bathing, skin/nail care, oral hygiene, and dressing.
• Record vital signs and report observations to the care team.
• Assist with meal planning, preparation, and feeding as needed.
• Travel to patient homes and deliver care as scheduled.
• Maintain accurate documentation of services provided.

Qualifications

Hospice Aide (CNA) Requirements & Responsibilities:

• Current CNA certification in the state of practice


• Valid driver's license, clean driving record, and auto insurance


• Provide personal care including bathing, skin/nail care, oral hygiene, dressing/undressing


• Record vital signs and report observations to the care team


• Assist with meal planning, preparation, and feeding as needed


• Promote patient comfort, dignity, and safety


• Travel to patient homes throughout the day and provide care as scheduled


• Maintain accurate documentation of services provided
