Job summary

St. Croix Hospice is seeking a compassionate Hospice Aide (CNA) to join our in‑home care team in Stevens Point, WI. You will work under an RN to provide one‑on‑one personal care and daily living assistance in patients’ homes, promoting comfort and dignity throughout end‑of‑life care.

The role requires current WI CNA certification, a valid driver’s license, and willingness to travel. You’ll join a dedicated interdisciplinary team delivering high‑quality care with a focus on compassion.

Qualifications

• Current CNA certification in the state of practice (WI)
• Valid driver’s license with clean driving record and auto insurance
• Provide personal care including bathing, grooming, and mobility assistance
• Record vital signs and communicate observations to the care team
• Record and maintain documentation of services provided

Responsibilities

• Deliver hands‑on personal and nursing care to patients at home
• Assist with activities of daily living and meals as needed
• Monitor patient comfort and safety and report changes to RN
• Travel to patient homes to provide scheduled care
• Maintain accurate patient records and documentation

Job Details: Job Location: Stevens Point, WI 54482, Position Type: Full Time, Travel Percentage: 100%, At St. Croix Hospice, we serve patients and families during one of life’s most important journeys—bringing expert care, dignity, and compassion to every home we visit.

As a Hospice Aide (CNA), you’ll be an essential part of our interdisciplinary care team, delivering compassionate, hands‑on care to patients in their homes. Working under the direction of a Registered Nurse, you’ll support patients with personal care and daily living activities, helping ensure comfort, dignity, and the highest possible quality of life at the end of life.

Qualifications: Hospice Aide (CNA) Requirements & Responsibilities:

• Current CNA certification in the state of practice
• Valid driver’s license, clean driving record, and auto insurance
• Provide personal care including bathing, skin/nail care, oral hygiene, dressing/undressing
• Record vital signs and report observations to the care team
• Assist with meal planning, preparation, and feeding as needed
• Promote patient comfort, dignity, and safety
• Travel to patient homes throughout the day and provide care as scheduled
• Maintain accurate documentation of services provided
