Responsibilities

The Hospice Aide provides personal care and related services in the home for the sick or disabled, assisting patients with at least 2 activities of daily living (ADLs). Responsibilities include:

• Assist with personal hygiene and at least one other ADL
• Help with medication intake and prescribed exercises
• Perform incidental household services essential to patient health care
• Follow and update the Plan of Care during visits
• Ensure use of the 4Ms (What Matters, Medications, Mentation, Mobility) and provide Age-Friendly Care

Qualifications

• Certified Nurse’s Aide (CNA)
• Valid driver’s license, reliable vehicle, good driving record
• High school diploma or GED preferred
• Ability to work within a multidisciplinary team and in field situations
