Job summary

Qualifications

• Registered Nurse with current license in the state of employment.
• 2 years hospice experience preferred.
• Ability to communicate effectively with patients, families and team members.
• Compassionate and caring demeanor while working with patients.
• Detail-oriented and observant; strong time management.

Responsibilities

• Observe and monitor patient conditions and perform assessments to develop an individualized care plan.
• Administer medications as prescribed and update the plan of care for comfort and symptom control.
• Prioritize visits for high-risk patients and manage after-hours and on-call needs.
• Educate patients and families using teach-back methods to ensure understanding.
• Complete clinical documentation following agency policy and Medicare guidelines.
• Collaborate with interdisciplinary team to ensure optimal care and regulatory compliance.
