Job summary

Advocate Health is seeking an experienced Hospice RN to provide after-hours nursing care in home settings. You will perform comprehensive assessments, triage, and interventions, prioritizing pain and symptom management aligned with hospice goals.

This role demands strong communication, crisis management, and independent decision-making, with duties including education for patients and caregivers and collaboration with the care team. Travel to patient homes is required.

Qualifications

• Completion of an accredited or approved nursing program.
• Active RN license in the state of practice.
• BLS certification – American Heart Association.
• Valid driver’s license.
• Typically one year of nursing experience.
• Experience in hospice, palliative care, oncology, ICU, ED, or home health is preferred.

Responsibilities

• Provide after-hours hospice nursing assessments, triage, and interventions.
• Respond to patient and caregiver calls; educate and determine interventions.
• Perform in-person home visits to manage acute symptoms.
• Administer physician orders and hospice protocols within scope.
• Document encounters accurately in the EMR and ensure continuity of care.
• Collaborate with physicians and the care team to adjust plans.
• Offer emotional support and guidance for end-of-life choices.
• Travel to patient homes and community settings as required.
