Job summary

AccentCare is seeking an After Hours Registered Nurse (RN) in Middlebury, CT. This full-time, on-site role supports hospice patients across New Haven and Fairfield County with an on-call rotation and rigorous nursing standards, including strong documentation and interdisciplinary collaboration.

The position requires an RN license, an associate degree preferred, CPR recommended for direct patient care, and the ability to lift and transport supplies as needed.

Qualifications

• Active RN license in the applicable state.
• Associate degree in Nursing preferred.
• CPR certification recommended for direct patient care.
• Must be willing to participate in on-call rotations and have reliable transportation.

Responsibilities

• Perform comprehensive nursing assessments and collaborate with IDG to develop care plans.
• Maintain accurate documentation and ensure plans reflect patient status; coordinate resources.
• Provide education and emotional support to patients and caregivers; participate in on-call rotations and care transitions.
• Coordinate care with administrators and supervise related staff when acting as Care Manager or in on-call roles.
