Job summary

Residential Home Health and Hospice is seeking an After Hours Licensed Practical Nurse to provide end-of-life care to patients in their homes in the Springfield, IL area. You will work with an interdisciplinary team to deliver compassionate care and support after hours.

Requirements include an active LPN license in the state, 2 years of LPN experience (hospice preferred), and a valid driver's license with approx. 20% travel to patient locations. Comprehensive benefits are offered.

What You'll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide after-hours support by conducting ongoing patient assessments focused on comfort, symptom management, and changes in condition, escalating clinical concerns to the RN as appropriate
• Educate and support patients and caregivers during evenings, nights, and weekends by reinforcing end-of-life care plans, symptom relief strategies, and what to expect throughout the hospice journey
• Ensure thorough, compliant EMR documentation

• Work independently, but never alone
• Travel to patient homes within assigned territory

After Hours LPN Qualifications

• Active Licensed Practical Nurse (LPN) license in the state you work
• Two years of LPN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
