Hospice of Central PA is seeking a Social Worker to join the Admissions Department in a hospital-based setting. This full-time role requires collaboration with clinicians and care teams to plan comfortable, patient-centered care and manage comprehensive assessments.

Qualifications include an MSW with LSW/LCSW eligibility within 2 years. A strong emphasis on transitions of care and community coordination is essential.

Qualifications

• MSW required.
• LSW or LCSW must be obtained within 2 years of hire.

Responsibilities

• Collaborates with hospice interdisciplinary team, hospitals, and care facilities to develop comfort-focused plans of care and complete assessments.
• Participates in hospital discharge planning to ensure safe transitions of care.
• Conducts joint home visits with admissions nurses when psychosocial complexity is present.
• Performs informational and follow-up visits for patients in hospitals, facilities, and home settings.
• Supports initial response for urgent referrals needing immediate attention.
