Hospice, Inc. is seeking a per diem RN to respond to hospice referrals and perform admissions across contracted facilities in the Hudson Valley region.

You will educate patients and families about hospice services, assess eligibility, and coordinate discharge planning with hospital staff. The role requires travel to hospitals within Ulster and Dutchess counties, weekend/holiday coverage, and strong communication with interdisciplinary teams.

Formación

• Graduate of an accredited nursing school with a valid NYS RN license.
• Certification in Hospice and Palliative Care or achieve within one year.
• Minimum of two years clinical nursing experience; hospice/home care/oncology preferred.
• Direct service experience in discharge planning, outreach and public relations preferred.

Responsabilidades

• Respond to hospice referrals and perform admissions per NHPCO guidelines.
• Assess coping, support systems and learning needs for arranging hospice services.
• Communicate with interdisciplinary team on end-of-life status and plan of care.
• Coordinate transfer from referral to admission with hospital and hospice staff.
• Identify referring physicians and case managers; document interactions in EMR.
• Initiate discussions on advanced directives and end-of-life care.
