Job summary

CareerNerds, LLC is seeking a Hospice Admissions RN in the Kansas City Metro, MO (east) area. This full-time position coordinates new patient admissions, conducts comprehensive assessments, educates patients and families, and partners with the interdisciplinary care team to ensure a smooth transition into hospice services.

The role requires Missouri RN licensure, CPR certification, and experience in clinical settings with hospice exposure.

Qualifications

• Current RN license in Missouri is required.
• CPR certification is required.
• Two years of RN experience in a clinical setting.
• One year of full-time hospice experience.
• Reliable transportation and auto insurance.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support continuity of care.
• Educate patients and families regarding condition and care needs.
• Perform head-to-toe physical assessments.
• Complete documentation accurately and on time.
• Perform other duties as assigned.
