Job summary

Essenti Talent is seeking a Hospice Admissions RN in the Kansas City Metro (east) MO area to coordinate new patient admissions and complete comprehensive assessments. You will educate patients and families, partner with the interdisciplinary care team, and ensure a smooth transition into hospice services while maintaining clinical excellence.

This full-time role requires Missouri RN license, CPR, and hospice experience; strong communication and independent decision-making are essential.

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Current driver's license and CPR certification.
• Two years of Registered Nurse experience in a clinical care setting.
• One year of full-time hospice experience.
• Reliable transportation and valid automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.
